Value-based Primary Care
Patient Family Advisory Councils (PFACs)
As the focus on value-based care continues, many primary care and specialty practices are developing Patient Family Advisory Councils (PFACs) as a way to partner with patients and family members to improve the quality of care, services provided, and the overall patient and family experience. Overseeing these councils can be time consuming, particularly for large systems with multiple practice sites.
A PFAC is an established council within a health care practice that consists of patients and family members who receive care at that practice along with members of the healthcare team. The council meets regularly to work on procedures, processes, and quality improvement strategies to achieve high-quality, coordinated patient- and family-centered care in the practice.
HCGC is a catalyst, convener, and coordinator of healthcare transformation and learning in the Columbus region and helps practices implement, facilitate, and engage PFACs in a neutral space.

Comprehensive Primary Care Plus (CPC+)
CPC+ is a national advanced primary care medical home model that aims to strengthen primary care through regionally-based multi-payer payment reform and care delivery transformation. CPC+ includes two primary care practice tracks with incrementally advanced care delivery requirements and payment options to meet the diverse needs of primary care practices in the United States.
HCGC is proud to be a subcontracted partner with The Health Collaborative in Cincinnati, Ohio to support practice transformation efforts for 162 CPC+ enrolled primary care practices in our region, from as far west as Lima, Ohio, extending through Central Ohio and into Southeast parts of the state.
